FIFA's Therapeutic Use Exemption (TUE) policy requires players and physicians to complete a detailed application form in one of the three official languages (English, French, or Spanish). The form must be completed in legible and complete form, and all medical files, including reports and documents, must be in one of the three official languages. The application form must be submitted by email or post, and incomplete or illegible applications will be returned and need to be resubmitted. The form includes sections for player information, previous TUE applications, and retroactive applications. Players who are part of the FIFA International Registered Testing Pool (IRTP) or the FIFA Pre-Competition Testing Pool (PCTP) must indicate their status on the form. The form also requires players to indicate whether they are participating in a FIFA competition or a national anti-doping organization (NADO) testing pool.
