These weaknesses are attributed to factors such as increased outsourcing of inspection and maintenance, pressure to keep assets in service, weak board-level leadership, reduction in manning levels, and poor operational experience and hazard awareness of personnel. To address these issues, it is recommended that organizations prioritize PSM performance, enforce systematic risk assessment, and engage the workforce in safety and operational excellence improvement initiatives.
